With all the noise and distraction of a reactive work environment, it is sometimes easy to overlook the obvious. For instance, if we wanted to perform a Root Cause Analysis (RCA) on an event, would we know which event was the most significant or costly? Experience demonstrates that we would not. In a reactive environment, we naturally become focused on the short term. We tend to look at the problems or events that just happened and naturally think they are the most significant at the time.
