Part 2 – Committing to Process Safety
The purpose of Part 2 of this book is to provide information that will help the reader understand the case for adopting risk‐based management practices in a LAPP. This information will help prevent process safety‐related incidents in a LAPP — incidents caused by the loss of control or containment of hazardous materials and energies.
Information is provided on basic risk management concepts with an emphasis on process safety culture, which is a key element for committing to process safety.
The incidents presented in Part 2 illustrate what has happened in a LAPP when appropriate hazard and risk analyses were not performed, when effective procedures and practices were not followed or not in place, and when hazards controls were ineffective or not implemented.
