APPENDIX F: HAZOP

Introduction:

HAZOP is a team approach involving a group of people representing all the concerned functions in a plant. The team work together to identify all the deviations by brainstorming to a set of guide words that are applied to all parts of the system. The team leader is an independent and competent individual trained in HAZOP who has the overall responsibility for conducting the HAZOP study. The HAZOP method was initially developed in the 1960s to analyse chemical process systems. Since then it has been applied to many other industries and has produced excellent results in enhancing safety, efficiency and reliability.
