High Reliability Organizations (HROs) measure risk. Then, they prioritize the risks and act to prevent failure. The tool they use is aptly named, Failure Mode, Effect, and Criticality Analysis (FMECA). FMECA was originally documented in 1949 by the U.S. military. (FMECA can be found in MIL Spec MIL-P-1629.)1
