e incident report is used to detect whether a prerequisite, product,

methodology, or lack of information is or has been a recurring error in

the process. If so, this should be xed with new or additional procedures.

Deviations can be detected when doing a job, conducting spot-checks,

or at quality meetings and during quality audits. Anyone who discovers

a discrepancy (the reporter) notes this in an incident report. e reporter

notes (if knowledge is available) if the discrepancy is due to his or her own

work or, if not, who is responsible for the deviation. e reporter may also
