When we started writing the second edition of this book, we thought we’d be able to approach it as most second edition authors have—refresh the statistics and update the observations from the first edition. We progressed to Chapter 3 before we realized that simply wouldn’t be enough. The entire landscape changed so much in the past 10 years that a content refresher would be a disservice.
