RCA METHODOLOGY
The term “proact”
has recently come to mean the opposite of react. This may seem
to be in conict with PROACT’s use as a Root Cause Analysis (RCA) tool. Normally
when we think of RCA, the phrase “after-the-fact” comes to mind—after, by its
nature, an undesirable outcome that must occur in order to spark action. So how can
RCA be coined as proactive?
In the last two chapters on Opportunity Analysis (OA), we clearly outlined a pro-
