Many of the breaches in the organization go unreported, either because it went undetected or the organization wants to save themselves from the embarrassment that comes from the bad reputation of the organization in front of the public.
An incident can be caused by human error as well. The security operations team might get a call in the middle of the night because some new deployment caused certain security functionalities to break within the server.
Incident response is one of the major responsibilities of the security team in most organizations. This increase in responsibility is directly related to the fact that the attacks are evolving—both in terms of volume as well as sophistication.
