If there is database corruption which cannot be soft-recovered (using the preceding method and the ;ntdsutil recovery command), you need to recover it from a backup.
In order to recover an AD database using a system-state-based backup, we need to use DSRM.
The relevant recovery steps using DSRM are explained on Chapter 11, Active Directory Services.
As I mentioned before, AD database issues are very rare in AD environments. If there are any, you most probably can recover from the situation using other options than restoring it from a backup. This should be the last resort in the troubleshooting process.
