Considerations
There are several considerations to keep in mind as you identify the demographic groups. Already mentioned was to avoid any groupings that appear to be ways to identify individuals; only ask for areas you will use, and only ask for areas applicable to your company.
Another consideration concerns equal treatment of employees. It is generally illegal to make employment decisions based on nonjob-required information such as race, sex, marital status, or military service. So if you identify any of these characteristics, use them solely to interpret survey results. Do not use any of this information to make employment decisions regarding an individual employee.
