Each training module has a cover sheet—a one-page document that contains information that both trainer and trainee need before proceeding with delivery of the actual training. Take a look at Worksheet 6.2 (page 98). Basic information on the cover sheet includes the following:
module number, title, and type (skill or knowledge)
performance objective
trainer preparation
special requirements
prerequisite modules, if any
procedure for training delivery
informative notes and purpose of task
names of module authors
name of an approval team member.
