1.To create a new template, choose File, New; click on My Computer… in the Templates
section of the New Document task pane; Select Blank Document on the General tab
and choose Template from the Create New box at the lower right of the Templates dia-
log box; click OK.
2.Put together the form’s static elements—the parts that won’t change each time the form
is used. This might include a logo, return address, tables, questions, and descriptions.
3.Use the Forms toolbar or the Control Toolbox toolbar to place form fields on the form.
