Placing blame is looking back to the past while taking responsibility is forward-looking. The focus should not be on whose fault something is but on helping to fix the problem.
If something could have been done better, rather than focusing on the negative software architects should seek to understand what lessons can be learned from the situation. Improvements can then be suggested so that a similar mistake does not happen again. Once the issue is resolved, move forward and do not dwell on it.
