These are captured in a structured way and are typically described using a unique identifier, a summary description, a category and type, the date the item was identified and who it was identified by, the impact, the priority, the resolution date, the owner, the person who is responsible for resolving or actioning the item, the strategy that was agreed on, the status, updates, and an escalation matrix.
Each of these items must be tracked until it is closed or resolved by the team.
