Besides creating natural fall-off (by moving the light really close to your subject), you can manually control fall-off by putting something over part of your softbox, so it covers the bottom, and then that part of the light from the softbox won’t fall on your subject. So, if you didn’t want to light their clothes or chest, you’d be set. We call this “flagging it off,” but I’ve also heard it called “goboing,” as in something that “goes between” the light and your subject (I’m a flagging guy myself).
