Especially when testing prototypes, it is good practice to clearly differentiate between tasks that require the participant to do something with the product being tested and questions (post-task or post-session) that are meant to elicit feedback on the experience. One way of doing this is to consistently prefix tasks with a "Task:" label and questions with a "Question:" label in the survey and add an explanation of this convention to the instructions.
